The story follows a new protagonist, 19-year-old Akira Maito, a former member of the GRC's elite squad, the "Rangaku." Akira's parents were killed in a ghoul attack when he was a child, and he was subsequently raised by his grandmother, a former ghoul who had defected to the human side. Akira's experiences have left him with a deep-seated hatred for ghouls, but also a strong sense of justice.
The CCG, led by the ruthless Kichimura Washuu (revealed to be the original One-Eyed Ghoul "Furuta"), engineers a catastrophic event: forcing Kaneki to undergo uncontrollable metamorphosis into a city-destroying Kaiju-like creature called "Dragon" . This event kills tens of thousands and turns sections of Tokyo into a monstrous wasteland. The narrative shifts toward a three-way conflict: Goat (Kaneki + allied Ghouls), the CCG (divided loyalties), and Furuta’s faction. Ultimately, Kaneki is rescued by Touka, regains control, and defeats Furuta. The series ends with Ghouls and humans beginning an uneasy peace, and Kaneki marrying Touka; they have a child, implying biological reconciliation between species. Ken Kaneki is presumed dead. The Commission of Counter Ghoul (CCG) has formed a special squad called the "Quinx" (Qs)—humans implanted with artificial Kagune (Ghoul predatory organs) granting them Ghoul-like abilities. Their leader is a white-haired, amnesiac investigator named Haise Sasaki (actually Kaneki with repressed memories). In his place stands Haise Sasaki, a First Class Ghoul Investigator and the mentor of the Quinx Squad—an experimental CCG unit composed of humans implanted with ghoul kakuhou (the organ that generates a kagune).
The final arc, "The Dragon," is often misunderstood. When Kaneki is captured and transformed by the CCG’s "Dragon" project, he becomes a city-destroying kaiju made of kagune. This is not a random escalation. It is the physical manifestation of suppressed trauma. The "Dragon" is every bad choice, every murdered friend, every drop of blood Kaneki refused to process exploding outward.
